What IMSI involves
The UK fertility regulator, the HFEA, describes IMSI as “a sperm selection method used in intracytoplasmic sperm injection (ICSI). The technique involves using a microscope to view detailed images of the sperm under very high magnification (over x6000) to select the sperm to inject into an egg.”
So IMSI does not replace ICSI; it changes how the one sperm injected into each egg is picked. The injection itself, and the rest of the IVF cycle, are unchanged.
An add-on, not a treatment on its own
The HFEA lists IMSI among treatment add-ons, which it defines as “optional non-essential treatments or tests that may be offered in addition to such proven fertility treatment”. It publishes its own rating of the evidence for IMSI, and this directory links to it rather than restating it.
It also notes, for all add-ons: “For most patients, having a routine cycle of proven fertility treatment is effective without using any treatment add-ons.” Whether it is appropriate in a particular case is a clinical decision for the treating doctor. This directory does not make that judgement.
What the regulator says about risks
The HFEA states: “IMSI is a non-invasive test performed on a semen sample as an additional step in the ICSI process. The risks associated with the use of ICSI also apply to IMSI. However, IMSI does not carry any additional known risks for the person undergoing fertility treatment or the child born as a result of fertility treatment.” The ICSI risks it refers to are quoted in full on the ICSI page.

Registry level, and why it matters here
India’s ART (Regulation) Rules, 2022 set out two levels of clinic:
A Level 1 clinic is one where “only IUI is carried out as part of treatment”.
A Level 2 clinic is one where “all/advanced investigations, diagnostic and therapeutic procedures in ART are carried out”.
Because a Level 1 clinic may carry out only IUI, IMSI is Level 2 work.
That is why each clinic below shows the level the national registry records for it. Of the clinics listed below, 29 are registered as Level 2, 3 as Level 1, and 18 have no level in the registry extract. This directory does not know why a Level 1 clinic names IMSI. A clinic may send patients to a partner Level 2 laboratory, or its website may describe services it does not provide on site. Ask the clinic where the IMSI itself is done, and check its registration on the National ART & Surrogacy Registry.

These counts come from reading clinic websites. Of the 4,801 clinics in this directory, 910 have a website on record and 894 of those have been read; 458 named at least one treatment.
